Discharge Information
As the Hospice only offers short term in patient facilities, we start planning discharges at an early stage
Ensuring a smooth discharge to home or nursing home takes planning. It is therefore necessary to begin discharge planning at an early stage in the patient’s admission. Following discharge patients may be referred to our Day Therapy Unit or Out Patient clinic. Hospice staff will also liaise closely with GP’s, Macmillan Nurses and District Nurses to ensure patients and carers are well supported at home. |
